The gift of nurturing support and care is one of the best gifts a new mother can receive. Thank you for desiring to gift her in this way! However, before I am able to serve her, this form must be completed by you. Afterwards, you and I must come to an agreed upon amount of service from the options I offer. The new mother must also fill out my Client Intake Form and meet with me in person prior to service so that I can get to know her better before serving her.
